New patients complete one set of forms online before the first visit — nothing to print and no clipboard at the front desk. Our team adds your answers to your chart. Printable copies are at the bottom of this page if you would rather use paper.

Fill out your forms online
One secure online form covers your information, medical history, a short symptom checklist and our financial policy. It takes about 15 minutes and goes privately to our office.
Need a printable copy? The online form is the easiest way — these copies are here just in case.
If you print at home, please bring the completed forms with you along with a photo ID and your insurance card — don't email them, since regular email isn't a secure way to send health information. Trouble printing? Call (225) 647-2294 and we'll have copies ready when you arrive.
