Plain-language summaries of how we handle your information and this website. Questions about any of it? Call (225) 647-2294.

Privacy Policy

This website collects only what you choose to give us — such as your name and contact details when you request a consultation. We use that information to respond to you, and for no other purpose. We do not sell or share your personal information with third parties for their marketing.

Please don't submit health details through this website; those belong in a phone call or the secure patient portal. Standard website analytics may collect anonymous usage data to help us improve the site.

To ask about, correct, or delete information you've submitted here, call (225) 647-2294 or email info@acwh.net.

Notice of Privacy Practices (HIPAA)

As a medical practice, we are required by law to maintain the privacy of your protected health information (PHI), to give you notice of our legal duties and privacy practices, and to follow the notice currently in effect.

We use and disclose PHI for treatment, payment, and health care operations. You have the right to inspect and receive a copy of your records, request corrections, request restrictions on certain uses, receive an accounting of disclosures, and file a complaint — with us or with the U.S. Department of Health and Human Services — without retaliation.

The complete current notice is available at our office. To request a copy or exercise any of these rights, call (225) 647-2294.

Telehealth Consent (Louisiana)

Telehealth visits at ACWH are conducted by secure, HIPAA-compliant video with a licensed Louisiana provider. You must be physically located in Louisiana at the time of your visit — a state licensing requirement.

Telehealth has limits: some conditions require an in-person exam, and if that's the case we will tell you plainly and arrange an office visit. Technical failures can interrupt a visit; if the connection drops, we call you and complete the visit by phone or reschedule at no disadvantage to you.

You may decline or stop telehealth at any time and choose in-person care instead. Your formal consent is documented in the patient portal before your first video visit. Telehealth is never for emergencies — call 911 in an emergency.

Terms of Use

If any part of this site is hard for you to use, please tell us: call (225) 647-2294 or email info@acwh.net, and we'll help you directly and fix the issue.
